Registration Fee:
General Registration is $125.00.  Included in the registration fee will be lunch and an evening reception on September 10, and a tour of the Spallation Neutron Source.  General registration will close on Tuesday, September 3 and Foreign National registration will close on Friday, August 23, 2019.

Workshop Description:

The workshop is an open forum for individuals affiliated with the United States Department of Energy or international accelerator facilities where each organization can present information/experiences on key operational and safety themes and share lessons learned.

You can register for the DOE Accelerator Safety Workshop 2019 here by using a credit card or request an invoice to pay by check. We accept Visa, MasterCard, Discover or American Express. An invoice/mailing check option is also available. An email confirmation will be sent upon completion of this form. To view your registration, print an invoice, reprint a receipt or submit payment, please use the login that you created to access this site.

Payment of fees must be in U.S. dollars drawn on a U.S. bank. 

Please note: The University of Tennessee Conferences and Non-Credit Programs is responsible for processing all registration fees for this event.

NOTE: University of Tennessee employees should register online and check-out of the site with a balance due.  Please send the departmental account number we should charge to email: Conferences@utk.edu.  Our office will initiate the payment. 

Please do not use a procurement card for your registration fee. If you will be asking for a UT reimbursement for this meeting, please do not register with a personal credit card, register with a departmental account to avoid additional processing fees.  For any credit card refunds processed due to not paying with a departmental account number there will be a 6% credit card processing fee based on the sale amount.  The 6% fee will be withheld from the refund to the card used. 

Program Questions/Refunds/Cancellation Policy:
Refunds will be subject to a $25 cancellation fee. NO REFUNDS allowed after August 23, 2019. Program questions should be directed to Jacob Platfoot, platfootjp@ornl.gov .
